Coxa retrotorsa – external femoral torsion

Orthopedic problems in children, especially those affecting the lower limbs, are among the conditions that require early detection and are a major concern for specialists.

During a child's development, there are certain aspects or postures of the lower limbs considered "physiological," which are most often transient and have no pathological significance. However, there are also deficiencies that should not be overlooked. The most important thing is early diagnosis. Early diagnosis of these conditions allows for treatments that can facilitate faster healing.

One of these deviations is coxa retrotorsa. What is coxa retrotorsa?

Retroversion refers to the angle of the femoral neck in relation to the femoral shaft. Femoral retroversion occurs when the neck of the femur is rotated backward. Most patients with pronounced retroversion walk with their big toes turned outward to avoid hitting the pelvis and causing pain. Excessive rotation may require surgical intervention, but in most cases it can be corrected through physiotherapy.

How can you spot the problem in children?

The easiest thing to notice is the way they walk. Walking with the toes pointed outward (the "Charlie Chaplin walk") can be a sign of femoral retroversion.

Another sign could be decreased internal rotation and increased external rotation at the hips. Other signs that may warrant a specialist check-up: if a child sits cross-legged and can easily bring their knees down to the supporting surface from there; if, regardless of the position they sit in, the child's toes point outward.
