What is tinnitus?

Tinnitus or ear ringing is usually a symptom of an ear disorder and can be a short-term problem or one that lasts for years.

Subjective tinnitus is the perception of a sound in the absence of an acoustic sound and is heard only by the patient.

Objective tinnitus is the result of noise generated by vascular tissue in the ear and, in some cases, is heard by the examiner.

Tinnitus can be described as a rumble, ringing, noise, roar, whistling, and is sometimes variable and complex. It can be intermittent, continuous, or throbbing (synchronous with the heartbeat).

Hyperbaric oxygen treatment

Hyperbaric oxygenation allows a controlled increase in oxygen pressure in the blood. This technique can be used in cases of tinnitus and sudden deafness, when certain changes in the inner ear and brain generate a lack of oxygen and, therefore, a limited intake of energy. The results obtained so far allow the recommendation of applying a high oxygen pressure in therapy when standard treatments fail. It can reach an improvement rate of 60-65% in cases of tinnitus. It is recommended to start hyperbaric oxygen therapy as soon as possible. Especially in cases of sudden hearing loss, success depends on the rapid application of HBOT. Thus, HBO therapy broadens the spectrum of treatment options for tinnitus and sudden hearing loss.

Although the pathophysiological reasons for tinnitus have not been fully explained, several findings indicate that it is often caused by a problem with blood transfer to the inner ear that affects the oxygen content of the cells that suffer. Probably the metabolism of the vascular streak is especially affected. This tissue sees the correct composition of the endolymph and thus protects the hearing cells. For this purpose, the presence of aerobic oxygen at high pressure is required.

The metabolism of anaerobic energy is less important. Together with the cells of the retina, the cells in the vascular tissues belong to the tissues with the highest concentration of oxygen in the human body. Therefore, blood flow is high in these tissues. Studies have shown that tissue in the basal corners of the cochlea consumes up to three to four times more oxygen.

Even though the method of determining the value of oxygen is now obsolete, it provides an important indication of the connection between a lack of oxygen and how the tissue reacts.
The close relationship between aerobic energy metabolism and inner ear function becomes evident through a high synthesis of ATP that influences sound waves.

With sounds of 89-90 dB, the oxygen pressure increases by 20% in the perilymph. An increase in blood circulation was also observed. However, a very high intensity reduces blood consumption and decreases the supply of deoxy-glucose. There is information that can also influence the neuronal impulses of blood flow in the cochlea.

The partial pressure of oxygen (pO2) in the endolymph is remarkably high. It was rated at 55-79 mmHg on average. With a normal pO2 of 90-95 mmHg in the blood, the difference in endolymph is distinctly smaller compared to the muscular system, where we found values ​​of 35-45 mmHg. The pressure difference, however, is, in addition to the diffusion stage, of central importance for the transfer of oxygen from the blood to the cell.

Hyperbaric oxygenation

The efficiency of hyperbaric oxygen therapy is based on increasing the partial pressure of oxygen in the blood and thus the pressure difference from the tissue. According to biophysical laws (gas laws), the pressure of oxygen in the blood depends on the pO2 in the inhaled air.

What are the effects of oxygen therapy on the body?

At an ambient pressure of 2.0-2.5 bar, a 5-8 times increase in blood pO2 and a 3-4 times increase in tissues can be expected. The necessary condition for oxygen therapy is intact lung tissue and normal heart and brain function. Stimulating microcirculation in damaged tissues must also be good enough to make sure they are getting enough oxygen.

A sudden dysfunction of the inner ear will probably cause an interruption of the blood supply to the vascular tissue and therefore to Corti's organ, which disrupts the functioning of the ear and generates negative results.
Tinnitus, hearing loss, and vertigo may occur separately or together, suddenly or as an aggravation of the chronic condition. The causes of vertigo, hearing loss, and tinnitus can occur as central or peripheral lesions.

Without exception, the patient should be examined immediately and appropriate treatment given.

During the consultation, the task of otoneurologists is:
  • to do detailed anamnesis (which can be 60% of the diagnosis)
  • to perform an ENT clinical examination
  • to perform a therapeutic examination
  • to do x-rays (Stenvers, Schuller, cervical spine)
  • to do CT
  • to perform a hemological examination (viscometry)
  • consult (if necessary) a neurologist, ophthalmologist, internal medicine specialist, orthopedist, or rheumatologist.

Strategy

  1. A complex examination of the condition.
  2. Adequate improvement of pathological parameters.
  3. Clinical follow-up.

Materials and methods 

The patients were selected for 9 months, between 1991 and 1992 from the ENT outpatient department of the Buda Military Hospital and it is intended to examine the pathological parameters and to what extent these parameters have been improved during the appropriate period of therapy. It also aims to determine the extent to which there has been a proven clinical improvement in various groups.

Patients: 30 people (25 men and 5 women). 
Average age: 50.3 years. 
Treatment of patients was necessary following acute complaints or sudden worsening of the corneal condition.
Efforts have been made to prevent toxicity and inflammatory manifestations. So, in addition to the usual ENT examination, neurological studies and a radiological examination were performed.

Exams

  • Total blood viscosity rrBVI at three-speed gradients (10, 40, 90 sec. I) with the help of heat-resistant HEVIMET-40 (37 °), computerized viscometer, which is suitable for mass measurement (HEMOREX, Budapest);
  • IPVI plasma viscosity tests at 37 ° with HEVIMET;
  • Serum triglycerides;
  • Serum cholesterol;
  • Hematocrit IhtI;
  • IFIBI fibrinogen according to the Klauss method (Multifibren, Behringwerke AG, Hamburg)
  • lRatelESRI.
All of these tests provide important information in vascular disease angiology and hematology. They must be used in ENT diseases, which have circulatory causes.

Results

The occurrence of pathological rheological values ​​occurred in 24 patients in the examined group. Normal hemorheological values ​​in the laboratory:
  • PV
  • TBV 10 sec- I
  • 40 sec- I
  • 90 sec- I
  • FIB 1.8-3.5 gil
  • Ht 0.37-0.45 gil
Such a large number of pathological values ​​can result from the relatively high average age of the examined patients and from a large number of risk factors (in 60% of cases there was diabetes, hypertension, hypercholesterolemia).

On the other hand, the occurrence of pathological changes of the cervical spine was found in 22 cases. In these cases, the following additional physiotherapy treatments were applied according to their conditions:
  • ultrasound;
  • iontophoresis;
  • massage;
  • therapeutic exercises;
  • balneotherapy.
The following differences were found in the 24 rheological pathological cases:
  1. Increased TBV in 6 patients with normal rheological hematocrit value (by 0.37-0.45 Ht). Crystallido solution with Vinpocetine (Cavinton, Richter Gedeon) 50 mg / day or Pentoxifyllin (Trental) 400 mg / day was infused.
  2. The common occurrence of TVB increase and Ht level over 0.45 in 12 patients. In these cases, IIHD / isovolemic hemodilution was applied (300 ml blood puncture by the simultaneous administration of 500 ml Gelifundol). This procedure was applied until the Ht value reached 0.37-0.45 because Ht improves the oxygen supply to the tissues by correcting the fluidity.
  3. Increased PV was seen in only 6 patients. Therefore, Natrium-pentosanpolysulfuricum (SP 54) 100 mg/day was combined with 500 ml of crystalloid solution for 10 days - with platelet count control - then changed to SP 54 50 mg tablets 3 times a day. . Under this treatment, not only did reduce PV, but it also decreased plasma fibrinogen.
Regarding the problems, 24 patients had tinnitus and of these, in 20 cases, had pathological hemorrhagic values. Patients received treatment (11 people IHD, 4 people SP 54, 5 received Pentoxifylline). As a result of treatment, tinnitus decreased in 11 patients and disappeared in 9 people. 24 patients had hearing loss. After treatment, hearing improvement by 5-10 dB was obtained only in certain cases but was not statistically significant. In some cases, patients explained that they could observe certain sounds and noises after treatment, while the demonstrable change in hearing threshold was not recorded. This phenomenon has been called "subjective" hearing improvement, in which improving cerebral blood circulation probably also plays a role. Despite the unchanged hearing threshold, the hearing experience becomes perfect, because there is better processing of sound information. 19 patients had pathological rheological parameters. These parameters became normal in 14 cases after treatment and in 10 of the 14 subjective hearing improvements.

In 10 cases, vertigo occurred, but the rheological values ​​were pathological in each of them. There were consecutive ENG signs of insufficient central blood circulation in almost all cases (eg, interrupted follow-up, eye movement, calorimetric asymmetry, dysrhythmia, writing problems).
